General Revisions
1.0 PURPOSE
The purpose of this procedure is to provide guidance for the proper implementation and use of Job Safety Analysis (JSAs).
2.0 SCOPE
Job Safety Analysis is required to be used on every Nitro Construction Services’ project. Sub-contractors to Nitro Construction Services are required to comply with the requirements of this procedure unless otherwise noted in contract documents. The only exception is if the client requires a specific safety analysis document. As long as pre-job planning is done, steps to do the job are detailed and safe work practices to mitigate risks are outlined, the JSA and Client’s document serve the same purpose.

4.0 RESPONSIBILITIES
4.1 Site Manager/Superintendent
4.1.1 Initiate the Job Safety Analysis process for all project work activities on their project(s).
4.1.2 Monitor the JSA process for effectiveness.
4.2 Supervisor
4.2.1 Monitor the JSA meetings prior to the start of the work
4.2.2 Review the JSA in the field for proper usage.
4.2.3 Train foremen in the proper use and completion of the JSA
4.2.4 Assist foremen in the effective implementation of the JSA process.
4.3 Foreman
4.3.1 Assess the specific task for safety considerations.
4.3.2 Assign work roles and responsibilities.
4.3.3 Train workers in the proper use and completion of the JSA.
4.3.4 Complete the JSA.
4.3.5 Review the information with the crew assigned to perform the work.
4.3.6 Note the comments or changes that arise from the review process.
4.3.7 Review the JSA with any employee who did not attend the initial review and obtain their signature on the form before performing work.
4.4 Worker
4.4.1 Review the information.
4.4.2 Sign the JSA indicating an understanding of the work and precautions to be taken.
4.4.3 Ask questions about any task or step about which they are unsure.
5.0 PROCEDURE
JSAs are designed to help the foreman identify and address the hazards associated with daily work assignments. JSAs require the foreman to review the hazards and the associated protective measures with the crew prior to tackling the daily tasks. Regardless of classification or severity of hazards, protective measures shall be listed for ALL hazards listed on the JSA. After receiving the review from the foreman each member of the crew has an opportunity to provide input or ask questions. Each crewmember then signs the JSA, indicating that he/she understands the sequence of work and precautions to be taken. Should an injury occur during the operation, a description of the injury or incident, including the at-risk behavior that caused the incident, shall be noted on the JSA. This data will aid in trend analysis and corrective action initiatives
5.1 JSAs shall be completed for all work tasks prior to the initiation of work activities.
5.2 A thorough JSA Briefing shall be held with all employees involved with the Task. Any workers joining the task at a later time shall have a separate and thorough briefing prior to starting the task.
5.3 Briefing shall include:
5.3.1 Overview of the hazard(s) involved in the task.
5.3.2 Controls taken to eliminate and/or reduce the hazard(s).
5.3.3 Asking questions of the crew about the task to ensure understanding of the information presented.
5.3.4 Documentation of the Crew’s understanding by the signature of workers involved.
5.4 The JSA shall be readily accessible (on their person or at the task location) for reference by the foreman and crew, and for review by the supervisor or the Safety Department.
5.5 Task completion section shall be completed, with any significant change in conditions or the work scope or plan noted on the JSA
5.6 Injury, environmental, and/or property damage events shall be noted on the associated JSA.
5.7 The foreman shall review the JSA for accuracy and sign for completeness at the end of the task.
5.8 The JSA shall be turned in at the end of the workday to a location designated by the Site Manager/Superintendent.
5.9 The JSA shall be reviewed for accuracy and completeness. This review may be performed by the supervisor, a member of the safety department, and/or the designated Safety Representative. The Site Manager retains the overall responsibility for this review being done in an effective, timely manner.
6.0 RECORDS RETENTION
6.1 Copies of the JSAs covering any injury, equipment damage, or near-miss report shall be included in the file for the incident and shall be sent to the Corporate Safety Director along with other documentation.
6.2 Job Safety Analysis shall be retained for a minimum of thirty (30) days at the job site.
